How to Create a RACI Matrix in 3 Steps

Use this sequence in a kickoff, process review or ownership workshop. Start with deliverables—not job titles.

List tasks and decisions

Write specific outputs such as “approve scope,” “test release” or “publish incident update.” Avoid broad phases such as “planning.”

Assign R, A, C and I

Add the people doing the work first, then name the final owner. Add consultation and updates only where they change the workflow.

Resolve gaps and export

Review every flagged row with the team. Download the final CSV, record the review date and revisit it when scope or staffing changes.

Use these rules before sharing the matrix

A complete grid is not automatically a useful grid. Read each row as a real handoff and ask whether the named people can act without another meeting.

  • One A:Normally assign one Accountable owner to each task or decision.
  • At least one R:Name who performs the work—not only who approves it.
  • Use C sparingly:Too many required consultations can create a decision bottleneck.
  • Name real roles:Use a team or job role that workshop participants can identify.
  • Add a review trigger:Revisit the matrix when scope, staffing or approval authority changes.

What does RACI stand for?

RACI stands for Responsible, Accountable, Consulted and Informed. Responsible performs the work, Accountable owns or approves the result, Consulted provides two-way input and Informed receives one-way updates.

Can I use this RACI matrix template in Excel or Google Sheets?

Yes. Select a preset or edit the matrix, then download the CSV. Open it directly in Microsoft Excel or use File → Import in Google Sheets. Task names, role names and assignments remain editable.

How many Accountable people should a task have?

A common RACI practice is one Accountable owner per task or decision. Zero Accountable assignments creates an approval gap; multiple Accountable assignments can make final authority unclear. The generator flags both cases.

What is the difference between Responsible and Accountable?

Responsible means “does the work.” Accountable means “owns or approves the outcome.” A task can have several Responsible contributors, but it should normally have one Accountable owner. One person may hold both roles when that reflects the real workflow.

How do I create a RACI matrix for an Agile project?

Start with deliverables such as backlog refinement, sprint planning, development, testing, acceptance and review. Add roles such as Product Owner, Scrum Master, Developers, QA and Stakeholders, then verify ownership row by row. Load the Agile preset to begin.

Can I use RACI for ITIL or change management?

Yes. For ITIL incident management, map logging, triage, investigation, communications, recovery and review. For organizational change, map impact analysis, approval, implementation, communication, training and adoption review. Both presets are included above.

What is the difference between RACI, RASCI and DACI?

RACI maps delivery responsibility. RASCI adds Support for people who assist the Responsible owner. DACI focuses on decisions using Driver, Approver, Contributors and Informed. See the framework comparison for selection guidance.
